Documents Checklist for Iran Visa
Mandatory Requirements: Passport:
So original Passport with the validity of minimum six months and minimum two blank pages for visa stamping. So attach all old passports (if any).
Personal Cover Letter:
So covering-letter from applicant stating his name, designation, passport number, purpose and duration of visit and to be duly signed by authorised signatory. The letter should be addressed to – The Visa Officer, Embassy of the Islamic Republic of Iran, New Delhi.
Additionally Note: Covering-letter should be on letter head if the applicant is a self employee & on plain paper if employee.
Please Note :
a) Additionally visa Approval Number: Visa Approval Number has to be obtained from Ministry of Tehran (Hotel can provide the same).
b) Embassy may call for interview.
c) Additional charges will be applicable as authorised agency can only submit the application.
Iran Visa Form:
One visa application form to be fill and sign from the applicant.
Photo Dimensions: So two recent passport size photographs with matt or semi matt finish, 80% face coverage with white background.
Additionally note: Photograph should not be more than 3 months old and should not be used in any of the previous visas.
Ticket: Ticket itinerary.
Medical Insurance: Original Medical certificate is require and medical has to be done through the authorised hospitals : Dr. Lal Path Lab/ Max Hospital or Apollo hospital. So the applicant should undergo the tests mentioned below:
a) HIV AIDS
b) T.B by Mantoux
c) Hepatitis B & C
Additional Note: So the certificate must have the contact details of the hospital or doctor as embassy may contact & verify.

Health regulation for passengers leaving India:- There is no health check requirement by Indian Government on passengers leaving India. However, persons leaving for a yellow fever infected area are advised in their own interest to get themselves vaccinated and to be possession of valid yellow fever vaccination certificates before they leave the country.
