Documents Checklist for Lithuania
Mandatory Requirements: Passport:
Original Passport with validity of minimum three months and minimum two blank pages for visa stamp
Please note:
a) So passports issued beyond 10 yrs will not be accepted by the Embassy.
b) So observation on the passport needs to be checked with The Embassy / VFS prior to the submission of application.
c) So attach all your old passports (if any).
Lithuania Visa Form:
Visa application form has to be filled online by the applicant.
Photo Dimension:
One recent passport size colour photographs with matt or semi matt finish, 60%-80% face coverage, white background and without border (Size: 35 mm x 45 mm)
Please note: Photograph should not be more than 3 months old and should not be used in any of the previous visas.
Personal Cover Letter:
Covering Letter from applicant on business letter head mentioning name, designation,passport number, purpose and duration of visit in brief. The letter should be duly signed by authorised signatory with company stamp and addressed to – The Visa Officer,Embassy of the Republic of Lithuania , New Delhi
Additionally Note :Covering Letter on Letterhead if applicant is Self Employed or on plain paper if Employed.
Hotel Bookings:
Hotel confirmation for the entire period of stay in Schengen area.
Additionally note: Hotel should match the ticket dates and maximum stay should be in Lithuania.
Flight Reservation:
Ticket Itinerary.
Medical Insurance:
Original & Copy of Medical Insurance for the stay duration amounting to 30,000 Euros.

Bank statements:
a) So personal ITR for last 3 years
b) So original Bank Statement for last six months with Bank Seal and Sign. Note: Bank statement Should be on A 4 Size Paper
c) So salary slip for last three months.
d) So Credit Card copy/Foreign Exchange endorsement/Traveller’s Cheque.
Proof of Employment:
Self Employed:
a) So company Registration Certificate / Certificate of Incorporation
b) So brief Company Profile
c) So articles of Memorandum (In case the applicant is MD/Director)
d) So proof of Proprietorship / Partnership (In case applicant is Proprietor / Partner in a firm)
Employed:
a) So proof of employment like Salary Slip or Appointment Letter
b) So NOC from Employer
Student:
a) So leave Sanction Letter from school
Parents have to provide ITR, Bank statement and NOC letter
