Documents Checklist of Slovakia Visa

Mandatory Requirements: Passport:
Original Passport with validity of minimum three months and minimum two blank pages for visa stamp 
Additionally note:
Passports issued beyond 10 yrs will not be acceptable in the Embassy.
So attach all your old passports (if any).

Personal Cover Letter:
Covering-Letter from applicant on business letter head mentioning name, designation,passport number, purpose and duration of visit in brief. The letter should be duly sign from the authorised signatory with company stamp and addressed to – The Visa Officer,Embassy of the Slovak Republic, New Delhi.
Please Note: Covering-Letter on Letterhead if applicant is Self Employed or on plain paper if Employed.

Flight Reservation : Ticket Itinerary

Insurances : Original Medical Insurance for the stay duration amounting to 30,000 Euros

Slovakia Visa Form:
So the visa application form to be filled online.

Photo Dimensions:
Two recent passport size colour photographs with matt or semi matt finish, 60%-80% face coverage, white background and without border (Size: 35 mm x 45 mm)
Please note: Photograph should not be more than 3 months old and should not be used in any of the previous visas.

Bank Statements:
a) So Original Bank Statement for last six months with Bank Seal and Sign. Note :Bank statement Should be on A 4 Size Paper.
b)Three months salary slip 
c)Credit Card copy/Foreign Exchange endorsement/Traveller’s Cheque

Accommodation Proof : Prepaid hotel voucher in original with seal and signature and payment receipt
Additionally note: Hotel should match the ticket dates and maximum stay should be in Slovakia

Authorisation Letter: So the applicant will have to submit an authorisation letter authorising Bookrumz to submit and collect documents.

Additionally Note: Personal appearance is require to submit the visa application & Embassy will advice the date of biometrics at the time of submission.
