Documents checklist of Czech Republic visa
Mandatory Requirements: Passport:
So original Passport with validity of minimum three months (from the date of return) and minimum two blank pages (Back to back) for visa stamp.
Please note
a) But the passports issued beyond 10 years will not be accepted by the Embassy
b) Handwritten passports will not be accepted.
c) So attach all your old passports (if any)
Visa Application Form:
Additionally one visa application form back to back, duly filled and signed. Refer to Signature on Czech Visa Application Form.
Please note : Applicant mobile number should be mentioned in visa form as Embassy / Consulate can contact the applicant at any point of time during visa processing.
Photo Dimension:
So two recent passport size coloured photographs with matt or semi matt finish, 60%-80% face coverage, white background and without border (Size: 35 mm x 45 mm)
Please note: Photograph should not be more than 3 months old , scanned/stapled and should not be used in any of the previous visas.
Personal Cover Letter:
Covering Letter from applicant on business letter head mentioning name, designation, passport number,purpose and duration of visit in brief. The letter should be duly signed by authorised signatory with company stamp and addressed to – The Visa Officer,Embassy of the Czech Republic , New Delhi
Please Note :Covering Letter on Letterhead if applicant is Self Employed or on plain paper if Employed .
Flight Reservation : Ticket Itinerary
Hotel Bookings : Hotel confirmation for the entire period of stay as per the tour itinerary.
Please Note: Hotel should match the ticket dates and maximum stay should be in Czech Republic.
Bank Statements:
a) Company’s and Personal ITR.
b) Six months salary slip.
c) Original Personal bank account statement (proof of sufficient financial means) of last three months with Original Seal and Sign from Bank. If someone is sponsoring the Trip then a affidavit is required for sponsorship along with his Bank Statement and National ID Proof Copy which has signature in it. E.G:- PAN Card, Note : Bank statement should be on A 4 size paper.
d) Credit Card copy/Foreign Exchange endorsement/Traveller’s Cheque.
Proof of Employment:
Self Employed
a) Company Registration Certificate / Certificate of Incorporation.
b) Brief Company Profile.
c) Articles of Memorandum (In case the applicant is MD/Director).
d) Proof of Proprietorship / Partnership (In case applicant is Proprietor / Partner in a firm).
e) Import / Export License (if applicable).
Employed
a) Proof of employment like Salary Slip or Appointment Letter / NOC from Employer.
Student
a) Leave Sanction Letter from school
b) Additionally parents have to provide ITR, Bank statement and NOC letter
Authorisation Letter: Applicant will have to submit an authorisation letter authorising Bookrumz to submit documents.
Passport Collection Process: So embassy has announced that representatives cannot collect the documents on behalf of applicant from Czech Republic Visa Application Centres except in following conditions :
a) Immediate family member (that is Parents, Children, Spouse)
b) One member of a family group collecting the passports of the whole family.
c) A person collecting on behalf of Government officials if authorised on the Government letter head or department.
If any of the above comes for passport collection they need to carry the following documents:
a) Original Government ID card
b) Original ICR
c) Original authorisation letter from applicant
However, applicants who do not wish to collect their documents in person & whose documents cannot be collected by one of the above, will get their passports back by courier at an additional cost of Rs. 300/- to the dispatch address mentioned in the courier form & Covering Letter which has to be submitted along with the visa application.
Additionally Note: –
a) So the prior Appointment is mandatory.
b) If applying for Multiple Entry Czech Affidavit Form is required on Company’s letter head.
Processing Time : Processing time for short term Czech Republic visa application is 10 working days. So please note that this is an indicative processing time only. Processing time may increase or decrease at the discretion of the Embassy of Czech Republic. Further to that, applications with the date of departure earlier than 8 calendar days (including Saturday and Sunday) after the date of submission will not be accepted.
